Information is collected through: · Our website, phone or email when you submit an enquiry · In the clinic when you register · During the treatment appointment with regard to your condition It may also be collected by one of your family members or friends when contacting us on your behalf. Personal information is also collected through reports or letters provided by you or any other healthcare practitioners with your consent. Information will be updated for instance in case of change of address or phone number. Your payment information (e.g. debit/credit card) will not be collected or stored by Physiotherapy Northwest as we do not take payments over the phone. Payments are processed in the clinic securely and privately by the third party payment processor we use.
