This policy sets out how Physiotherapy Northwest Limited collects and uses any personal information it receives from patients, other healthcare professionals and health insurance companies under the GDPR. This policy applies to information we collect through our website as well as on paper in the clinic. By using our website, you consent to this policy. We are registered with the Information Commissioner’s Office for this purpose.

Information We Collect

We collect personal information, which can be used to identify you such as, name date of birth, home address, telephone number, email address, medical insurance membership and authorisation numbers as well as your GP details and the condition you are being treated for.

Information is collected through:

·      Our website, phone or email when you submit an enquiry

·      In the clinic when you register 

·      During the treatment appointment with regard to your condition

It may also be collected by one of your family members or friends when contacting us on your behalf. 

Personal information is also collected through reports or letters provided by you or any other healthcare practitioners with your consent. Information will be updated for instance in case of change of address or phone number.

Your payment information (e.g. debit/credit card) will not be collected or stored by Physiotherapy Northwest as we do not take payments over the phone. Payments are processed in the clinic securely and privately by the third party payment processor we use. 

Use and Disclosure of Your Information

We may hold and process personal data that you provide to us in accordance with the DPA and GDPR.

The information that we collect and store relating to you is primarily used to enable us to provide you with our services. We may use the information for the following purposes:

·      To notify you about your appointment booking and reminder, change or cancellation of appointment 

·      To email you physiotherapy statements or any relevant information with regard to your treatment

·      To notify you about change of clinic location or contact details when applicable

·      To send medical reports or referral letters with your consent to other healthcare professionals when applicable

·      To submit invoices or share clinical notes with medical insurance companies you are registered with, or any other third parties, where you have given us consent to do so as well as other healthcare professionals in case of referrals and medical reports.  

·      In addition, we may disclose personal information if we are required to do so by law, in connection with any legal proceedings, and in order to establish, exercise or defend our legal rights.

Your Rights

You have the right to:

  • Request information about how your personal data is processed and obtain a copy of that personal data
  • Correct anything inaccurate in your personal data 
  • Raise an objection about how your personal data is processed
  • Request that your personal data is erased if there is no longer a justification for it

Use of Cookies

Cookies are small files that are stored on your computer’s hard drive that track and save your interaction and usage of the website. When you visit our website, it may gather information about your computer to provide statistical information regarding the use of our website. Such information will not identify you personally. All computers have the ability to decline cookies. This can be done by activating the setting on your browser that enables you to decline the cookies.

Some areas of our premises are equipped with CCTV. Where in use, CCTV cameras are there for the protection of employees and third parties, and to protect against theft, vandalism and damage to the goods and property of the premises where we are based. Generally, recorded images are not shared with third parties unless there is suspicion of a crime, in which case they may be turned over to the police or other appropriate government agency or authority.

In case of any changes to this policy, the “last updated” date on this page will be updated accordingly. Any changes will come into effect immediately.
